Matching Gifts

Increasing your gift is simple -- just get a gift application from your personnel department. If it states that your company matches gifts or provides support to health organizations through one of the above programs, fill out the application and send it to us, along with your gift. Even if you've already sent CCFA your donation, you can still send us the completed form for processing:

Once we receive this form, we will process and return it to the appropriate organization or individual, who will then send us a check for the specific amount. Employee spouse and retiree gifts are often eligible as well. Every bit of support makes a tremendous difference to our efforts.
